Services Plan Pays
Physician Office or Outpatient Treatment Pays $100 per covered person up to annual maximum of $250 Single / $1,000 Family
Hospital Admission Benefit Pays $500 for the first day in hospital IN ADDITION to daily in-hospital benefit
Daily In-Hospital Benefit / Hospital Confinement Pays $250 per day (up to 180 days per confinement)
Hospital Intensive care Pays $250 per day in hospital intensive care IN ADDITION to daily in-hospital benefit
Surgical Benefit Pays up to $2,500 (Based on fee schedule)
Anesthesia Benefit Pays up to $625 not exceed 25% of amount paid under surgical benefit
Well Baby Care Pays $25 per visit (up to 4 visits per year). Baby is dependent child 12 months or younger
Prescription Drug Benefit Pays up to $20 per prescription. Limited to 5 prescriptions per year.
